Zoltral - GENERIC ZOLOFT
- is a selective serotonin reuptake inhibitor (SSRI) used to treat depression, panic disorder, obsessive-compulsive disorders
(OCD), post-traumatic stress disorder (PTSD), and a severe form of premenstrual syndrome (premenstrual dysphoric disorder -
PMDD). It may also be used to treat other conditions as determined by your doctor. Generic Zoloft works by helping to restore the balance of certain natural chemicals in the brain.
Zoloft is used to treat the symptoms of depression. It works by allowing nerves to communicate longer with each other through the presence of
serotonin. It inhibits the reuptake of serotonin from the area between nerves (synapses).

Mechanism of action
The mechanism of action of sertraline is presumed to be linked to its inhibition of CNS neuronal uptake of serotonin (5HT).
Studies at clinically relevant doses in man have demonstrated that generic Zoloft blocks the uptake of serotonin into human platelets.
In vitro studies in animals also suggest that sertraline is a potent and selective inhibitor of neuronal serotonin reuptake
and has only very weak effects on norepinephrine and dopamine neuronal reuptake. In vitro studies have shown that sertraline has no significant affinity for adrenergic (alpha1, alpha2, beta),
cholinergic, GABA, dopaminergic, histaminergic, serotonergic (5HT1A, 5HT1B, 5HT2), or benzodiazepine receptors; antagonism of such receptors has been hypothesized to be associated with various
anticholinergic, sedative, and cardiovascular effects for other psychotropic drugs. The chronic administration of
generic Zoloft was found in animals to downregulate brain norepinephrine receptors, as has been observed with other clinically effective antidepressants.
Generic Zoloft does not inhibit monoamine
oxidase.

What substances interact with Zoloft?
Some medicines or medical conditions may interact with
generic Zoloft. Inform your doctor or pharmacist of all prescription and over-the-counter medicine that you are taking.
Do not take generic Zoloft if you are also taking pimozide, weight loss drugs (e.g.,
fenfluramine, sibutramine), tryptophan, terfenadine, astemizole, dihydroergotamine, or monoamine oxidase inhibitors -
MAOI (e.g., furazolidone, isocarboxazid, linezolid, moclobemide, phenelzine,
procarbazine, selegiline, tranylcypromine). If you are currently taking any of the medicines mentioned above, tell your doctor or pharmacist before starting
generic Zoloft. Additional monitoring of your dose or condition may be needed if you are taking
warfarin, lithium, tricyclic anti-depressants (e.g., amitriptyline), other SSRI anti-depressants (e.g.,
citalopram, fluvoxamine), nefazodone, venlafaxine, propafenone, flecainide, "triptan" migraine drugs (e.g.,
sumatriptan), carbamazepine, thioridazine, trazodone, ayahuasca, St. John's wort, clozapine, tramadol, melatonin,
dextromethorphan, meperidine, and buspirone.

Side effects of Generic Zoloft
Some people taking
generic Zoloft may have side effects. The most common side effects include upset stomach, having trouble sleeping, diarrhea, dry mouth, sexual side effects,
feeling unusually sleepy or tired, tremor, indigestion, increase of sweating, feeling agitated, and decreased appetite.
In clinical studies with Zoloft, few patients were bothered enough by side effects to stop taking their medicine. Tell your doctor about any side effects you
may be experiencing. Your doctor may be able to adjust your treatment plan to help you feel better.

Cautions
Several weeks may
pass before generic Zoloft reaches its full effect. Do not stop taking
generic Zoloft without checking with your doctor. DO NOT DRIVE, OPERATE MACHINERY, OR DO ANYTHING ELSE THAT COULD BE DANGEROUS until you know how you react to
generic Zoloft. Using generic Zoloft alone, with other medicines, or with alcohol may lessen your ability to drive or to perform other potentially dangerous tasks.
Generic Zoloft WILL ADD TO THE EFFECTS of alcohol and other depressants. Ask your pharmacist if you have questions about which medicines are depressants.
BEFORE YOU BEGIN TAKING ANY NEW MEDICINE, either prescription or over-the-counter, check with your doctor or pharmacist. This includes any medicines that contain
dextromethorphan. FOR WOMEN: IF YOU PLAN ON BECOMING PREGNANT, discuss with your doctor the benefits and risks of using
generic Zoloft during pregnancy. IT IS UNKNOWN IF generic Zoloft IS EXCRETED in breast milk. IF YOU ARE OR WILL BE BREAST-FEEDING while you are using
generic Zoloft, check with your doctor or pharmacist to discuss the risks to your baby.

Generic Zoloft dosage and administration
Follow the directions for using
GENERIC ZOLOFT provided by your doctor. It is recommended you take your dosage at the same time each day, either in the morning or in the evening.
Take this medication regularly in order to get the most benefit from it. Continue to take geneic Zoloft even if you feel better. Store
generic Zoloft at room temperature at 77 degrees F (25 degrees C) away from light and moisture. Brief storage between 59 and 86 degrees F (15 and 30 degrees C) is permitted. Do not store in the bathroom.
If you miss a dose of GENERIC ZOLOFT, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule.
Do not take 2 doses at once.
Initial Treatmentt
Dosage for Adults
Depression and Obsessive-Compulsive Disorder: generic Zoloft treatment should be administered at a dose of 50 mg once daily.
Panic Disorder: generic Zoloft treatment should be initiated with a dose of 25 mg once daily. After one week, the dose should be increased to 50 mg once daily.
While a relationship between dose and effect has not been established for depression,
OCD, or panic disorder, patients were dosed in a range of 50-200 mg/day in the clinical trials demonstrating the effectiveness of sertraline HCl for these indications. Consequently, a dose of
generic Zoloft 50 mg, administered
once daily, is recommended as the initial dose. Patients not responding to a 50 mg dose may benefit from dose increases up to a maximum of 200 mg/day. Given the 24 hour elimination half-life of sertraline
HCl, dose changes should not occur at intervals of less than 1 week.
Generic Zoloft should be administered once daily, either in the morning or evening.
Dosage for Pediatric Population (Children and Adolescents)
Obsessive-Compulsive Disorder: generic Zoloft treatment should be initiated with a dose of 25 mg once daily in children (ages 6-12) and at a dose of 50 mg once daily in adolescents (ages 13-17).
While a relationship between dose and effect has not been established for
OCD, patients were dosed in a range of 25-200 mg/day in the clinical trials demonstrating the effectiveness of sertraline HCl for pediatric patients (6-17 years) with
OCD. Patients not responding to an initial dose of generic Zoloft 25 or 50 mg/day may benefit from dose increases up to a maximum of 200 mg/day. For children with OCD, their generally lower body weights compared to adults should be taken into consideration in advancing the dose, in order to avoid excessive dosing. Given the 24 hour elimination half-life of sertraline
HCl, dose changes should not occur at intervals of less than 1 week.
Generic Zoloft should be administered once daily, either in the morning or evening.
Dosage for Hepatically Impaired Patients
The use of generic Zoloft in patients with liver disease should be approached with caution. The effects of sertraline in patients with moderate and severe hepatic impairment have not been studied. If
generic Zoloft is administered to patients with liver impairment, a lower or less frequent dose should be used.
Maintenance/Continuation/Extended Treatment
Depression: It is generally agreed that acute episodes of depression require several months or longer of sustained pharmacologic therapy. Whether the dose of antidepressant needed to induce remission
is identical to the dose needed to maintain and/or sustain euthymia is unknown. Systematic evaluation of sertraline HCl has shown that its antidepressant efficacy is maintained for periods of up to 44 weeks
following 8 weeks of open-label acute treatment (52 weeks total) at a dose of
generic Zoloft 50-200 mg/day (mean dose of 70 mg/day).
Obsessive-Compulsive Disorder and Panic Disorder: Although the efficacy of sertraline HCl beyond 10-12 weeks of dosing for OCD and panic disorder has not been documented in controlled trials, both are
chronic conditions, and it is reasonable to consider continuation of a responding patient for either indication. Dosage adjustments may be needed to maintain the patient on the lowest effective
dosage of generic Zoloft, and patients should be periodically reassessed to determine the need for continued treatment.
Switching Patients to or from a Monoamine Oxidase Inhibitor
At least 14 days should elapse between discontinuation of a MAOI and initiation of therapy with sertraline
HCl. In addition, at least 14 days should be allowed after stopping sertraline HCl before starting a
MAOI.

CLINICAL STUDIES
Depression
The efficacy of sertraline HCl as a treatment for depression was established in two placebo-controlled studies in adult
outpatients meeting DSM-III criteria for major depression. Study 1 was an 8-week study with flexible dosing of sertraline
HCl in a range of 50 to 200 mg/day: the mean dose for completers was 145 mg/day. Study 2 was a 6-week fixed-dose study,
including sertraline HCl doses of 50, 100, and 200 mg/day. Overall, these studies demonstrated sertraline HCl
to be superior to placebo on the Hamilton Depression Rating Scale and the Clinical Global Impression Severity
and Improvement scales. Study 2 was not readily interpretable regarding a dose response relationship for
effectiveness.
Analyses for gender effects on outcome did not suggest any differential responsiveness on the basis of sex.
Obsessive-Compulsive Disorder (OCD)
The effectiveness of sertraline HCl in the treatment of OCD was demonstrated in three multicenter
placebo-controlled studies of adult outpatients (Studies 1-3). Patients in all studies had moderate to severe OCD
(DSM-III or DSM-III-R) with mean baseline ratings on the Yale Brown Obsessive-Compulsive Scale (YBOCS) total
score ranging from 23 to 25.
Panic Disorder
The effectiveness of sertraline HCl in the treatment of panic disorder was demonstrated in three double-blind,
placebo-controlled studies (Studies 1-3) of adult outpatients who had a primary diagnosis of panic
disorder (DSM-III-R), with or without agoraphobia.
